Electron microscopic observations on infantile hypertrophic pyloric stenosis




Abstract
Hypertrophic pyloric stenosis is one of common surgical diseases of infants. Non bile stained projectile vomiting, visible gastric peristalsis and RUQ palpable mass are pathognomic signs. But its etiology and pathogenesis are still subjects of
speculation. There is no good evidence that the tumor represents work hypertrophy of the pyloric antrum secondary to repeated pylorospasm; but neither has this theory ever been disproved. Rather, it is thought to be inherently unlikely.
The pylorus obtained by biopsy at pyloromyotomoy in 14 infants with hypertrophic pyloric stenosis were studied by electron microscopy at Chonnam University Hospital in 1990.
There was a marked increase in overall thickness of the circular smooth muscle layer. It was composed of light appearing fibers with less compact myofilaments and darker fibers with more compact myofilaments. The nuclei and organelles did not
appear
abnormal. However, there was an increase in the number of subsarcolemmal pinocytic vesicles which often were present in multiple layers. No specific alternations were found in muscle fibers of the pylorus. So a functional etiology of this disease
is
suggested.
